isqlplus did not exist in V9 & does not exist V11.

I suggest you avoid isqlplus & use a real SQL client

Are you SURE it is iSQL*Plus? URL (//127.0.0.1:8080/apex) suggests it is APEX.

What caused this error? What did you do? Click - what exactly? Could you specify Oracle and Windows versions you use?

Quick Googling says that the problem might be a compatibility mode, or that you should create a desktop shortcut (instead of running the application from the Start menu). There are other results, but I guess you might check them yourself.
