This problem does occur when you perform an upgrade of the database, don't set _system_trig_enabled=false in init.ora. If you have this, you have probably duplicate procedures, one under SYS the other under a different user. This usually results in problems like you posted. Resolution: drop the non-sys copy of the procedure.
