Lsof and fuser are useless for finding out the swap allocations. Your best bet is something like gtop or gpm (glance X11). Alternatively, you can try reconstructing memory maps for each process. That is usually either in /proc or you'll need a special routine like "crash" or kernel debugger. In both cases, it's a very dangerous thing to do.
