The level of English depends on size of city. In small places it's not very common. Often youngers speak english at least a little. Sometimes people embarrassed of their poor English, but they understand what you say.
It helps to mix both languages, although sounds funny. :) On the phone, it's better to get someone who speaks english because when they start speak German, is's rude asking for someone else (happened several times :( ).

Some (really few) people refuse to speak English although I know they can. Could be a question of origin? :)
English is not my native language as well. Comparing to the US: It's much better to be "hardly-english-speaking" person in America than "hardly-german-speaking" in Germany. At least, I felt comfortable not understanding much English in US, but in Germany that bothers me and makes me kind of miserable.

It took half a year to get comfortable with how German sounds, but intonations and emotions are still hidden from me.

In IT, English is almost native language. In big IT companies that's the main language of communication. However, sometimes there are difficulties to speak with users.

It seems in Amsterdam there is no problem with language - almost everybody speaks English (and French and German :)
