mkfifo mypipe
(ORACLE_SID=source; export ORACLE_SID; exp system/manager file=mypipe blablah) & ORACLE_SID=target
export ORACLE_SID
imp system/manager file=mypipe full=y blahblah

If you understand how it works, well, nothing is much different from doing it in sequence (except the load for your machine ...). You can also use system/manager_at_source etc. if you do not want to toy with ORACLE_SID or if your databases are on different computers (and you have a a lot of bandwidth in between).
A bit like tar cvf - . | (cd elsewhere; tar xvf -), except that you have to explictly create the pipe.
