how to build a Manual Matrix Report, that extract data from a form not a table ?

How did data come into a form? From a table? A view? Stored procedure? FROM clause? Something different?

Actually, it doesn't really matter as I'd suggest you to store data into the database and then write a report to display these records. Because, I don't see (an easy) way to transfer records from a form directly into a report.
