I mean use oracle developer suite 9i or later to view the source code.

4.5 can be opened by Forms 5, 6, and 6i. 9i and 10g can not open 4.5 due to the change from PL/SQL V1 (in 4.5) to PL/SQL V2 (in 5 and 6) to PL/SQL V9-10 (in 9 and 10). You will have to either do a batch convert of the 4.5 forms using the f60/f65 executibles, then again using the f90 executibles, or open the form in 6i, save it, and then open it in 9i.

Typically you do a 4.5 to 6i conversion, add object groups, move the 'label literal' to 'item prompts'. remove any cyclic reference in your Form Libraries (pll files). Then do a 6i to 9 conversion.

Use, at least, 6i patchset 13 for your 6i conversion.
