Service request? I might be wrong, but - you don't "download" it, you put a request on My Oracle Support. See Creating Service Requests.

Thank you for your reply, @Littlefoot.
My English so bad to make you understand exactly from my question. I'm sorry

I meant where is Oracle Application (image attached) to download? From Oracle Application, I can update or raise a service request instead of logging to Oracle Support website. So, I found it from googling, please correct me if I'm wrong. The Oracle Application in which raises a service request is Oracle Application Express?

Unfortunately, I can't view images you posted (company policy doesn't allow me to); if possible, upload them to OraFAQ and attach them to your next messages directly, instead of posting a link to one of file sharing services (otherwise, I'll be able to see them much later today, when I return home).

There might be an application which allows you to raise a service request without logging to My Oracle Support, but I don't know whether it exists or not. Sorry, someone else will have to assist you with that.

As of Oracle Application Express (Apex) you mentioned: you can put a "service request" for Apex as described here (when you request a new schema, additional space storage or you want to terminate a workspace). However, I'm not sure that this is "service request" you are talking about.

I'm sorry for inconvenience.
I attached file here. Because forum's rule does not allow zip or rar file upload, so I change the OAE.rar to OAE.rdf, please unchange the name into .rar extension.

I'm not sure OAE got a same function to Oracle Support Service website, too, so I place my question here.

Thank you

Right; I've checked those images, thank you. However, I'm not familiar with that application so you'll really have to wait for someone else to assist.
