Only Yesterday I got this issue frequently. Otherwise it is very very rare.
Even though the posts are not updated again, I got 2 to 3 notifications for the same reply / post at the same time.

2 times
Have they duplicate posts and somebody cleaned them up?
(Or) Any issue with Notifications cron?
