Who is "Tim"?

Do these files really exist?
'/u01/oracle/oradata/qamfg/control03.ctl'
'/u03/oracle/oradata/dev/control01.ctl'

You shouldn't be worried about who is Tim,

Do these files really exist?
'/u01/oracle/oradata/qamfg/control03.ctl'
'/u03/oracle/oradata/dev/control01.ctl'

Please answer if you can?

Oracle reports problem & you claim all is well.
I trust Oracle to better report reality & discount your claims to the contrary.

The answer is there:

Linux Error: 5:Input/output error

Check your OS logs to know the reason.
